PHOTO AND VIDEO RELEASE

I hereby grant Madison Contemporary Dance the right to use photographs and videos taken of my child during classes, performances, and related events for promotional materials, including but not limited to the Madison Contemporary Dance website, social media, and advertising.

Model Release (For Minors):

For good and valuable consideration, I, the undersigned parent or legal guardian, grant Madison Contemporary Dance and its authorized representatives the irrevocable right to:

• Copyright and use photographs or videos in which my child may appear.

• Publish or re-publish these materials, in whole or in part, in any format (print or digital), without restriction on edits or alterations, for promotional purposes, including but not limited to advertising, editorial use, social media, and publications.

  
  

I understand that these materials may be used in conjunction with other images or media, with or without identifying my child by name. I waive any right to review or approve the final product, including any written or spoken copy related to the use of these materials

I acknowledge that this release covers all media formats currently known or developed in the future and may be used worldwide for an unlimited time. I also waive any claim of compensation, royalties, or any other payments related to the use of these materials.

Furthermore, I release and hold harmless Madison Contemporary Dance and its representatives from any liability, including claims of defamation, invasion of privacy, or misuse of image, related to the authorized use of these photographs or videos.

Parent/Guardian Consent for Minors: I, the undersigned parent or legal guardian of the minor named below, consent to the above on behalf of my child. I agree that this release is binding on me, my child, and our legal representatives.

RELEASE OF LIABILITY FOR MINOR PARTICIPANTS 

IN CONSIDERATION OF my child/ward named above being allowed to participate in any way in Madison Contemporary Dance programs, performances, and related events and activities (collectively referred to as "Releasee"), the undersigned acknowledges, appreciates, and agrees that:

1. The risks of injury and illness (including but not limited to communicable diseases such as MRSA, influenza, and COVID-19) to my child from participating in these programs are significant. These risks may include permanent disability and death. While certain rules, safety equipment, and personal discipline may reduce these risks, the possibility of serious injury and illness still exists.

2. FOR MYSELF, MY SPOUSE, AND MY CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, even if they arise from the negligence of the Releasee or others. I accept full responsibility for my child’s participation in these programs.

3. I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I notice any unusual or concerning issue regarding my child’s readiness for participation or the safety of the program itself, I will immediately remove my child from participation and bring the concern to the attention of the nearest official.

4. On behalf of myself, my spouse, my child, and our heirs, assigns, personal representatives, and next of kin, I HEREBY RELEASE AND HOLD HARMLESS Madison Contemporary Dance and Synergy Dance Academy, along with their directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, the owners and lessors of premises used to conduct the activities, FROM ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property arising from my child’s involvement or participation in these programs, even if caused by the negligence of the Releasee, to the fullest extent permitted by law.

5. I, on behalf of myself, my spouse, and my family, further agree to INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities related to my child’s participation, EVEN IF ARISING FROM THE NEGLIGENCE of the Releasees, to the fullest extent allowed by law.

   
   

By signing this agreement, I confirm that I have fully read and understood its terms. I sign it freely and voluntarily without any inducement. As the parent/guardian with legal responsibility for this participant, I have explained the risks and responsibilities outlined in this waiver/release to my child/ward. I acknowledge their personal responsibilities for adhering to program rules and measures to protect against communicable diseases. I agree to this release on behalf of my child/ward and also agree to indemnify and hold harmless the Releasees from any and all liabilities as described above, even if it arises from their negligence.
